Our Heartland Village philosophy. Elmcroft enhances the quality of life for each resident by recognizing that each person is an individual with unique talents, abilities, and life history. Our mission is to enrich lives by supporting independence, autonomy, and preferences for how care is delivered, and to provide meaningful, rewarding and engaging activities. We create a village that feels like home, with familiar people and objects, comfortable surroundings, and pleasant sounds and aromas. Elmcroft’s Care Coordinators approach each resident’s needs individually, based on that person’s preferences for how care is to be provided. Our families are an integral part of the care process, providing a partnership in the disease process and caring for their loved one. Elmcroft provides education to our Care Coordinators and to our families in order to create a better understanding of the disease process and how to meet the needs of each resident. At Elmcroft, we make every residents present as meaningful as their past. |
